Daily COVID-19 Questionnaire for Employees and City Hall Guests

Is your temperature above 100.4F?

Have you been tested for COVID-19 in the last 3 days?

If you are tested positive OR you are still awaiting your results, you should stay home.

If you tested positive for COVID-19 and have symptoms, it's critical that you self-isolate (stay in your home and away from others) for:

  • 10 days after symptoms first appeared and
  • 24 hours with no fever
  • If you tested positive for COVID-19 and have no symptoms: 10 days after you received your positive test results

If you are a close contact and have not quarantined/finished quarantining, you should stay
home.

Have you recently traveled outside of NJ, NY, CT, PA, or DE?

Depending on current travel advisory guidelines, if you have traveled recently, you may need to stay home for 2 weeks.

Please verify if, within the last 14 days:

Did you answer YES to any of the following questions:

1. Is your temperature above 100.4F?
2. Have you been tested for COVID-19 in the last 3 days?
3. Have you been in close contact with a positive COVID-19 person?
4. Do you have any of the listed COVID-19 symptoms?

IMPORTANT!

If you tested positive for COVID-19 and have symptoms, it's critical that you self-isolate (stay in your home and away from others) for:
· 10 days after symptoms first appeared and
· 24 hours with no fever
If you tested positive for COVID-19 and have no symptoms: 10 days after you received your positive test results. 
IF you are a close contact of someone who has tested positive for COVID-19: Quarantine at home; length of quarantine will be determined by the City of Clifton Administration and the Clifton Health Department.

No Test Option: Stay home for 10 days. If you stay symptom-free for 10 days, quarantine ends. Monitor symptoms, wear a mask, and social distance through day 14.

​Test Option: Stay home for 7 days. Get tested on Day 5, 6, or 7. Stay home while awaiting results. If the test is negative and you stay symptom-free, quarantine ends. Monitor symptoms, wear a mask, and social distance through day 14.
